We describe the epidemiology and treatment of the increasing problem of alcohol consumption in Sweden. The main causes of disease are alcohol abuse, alcoholism and alcohol abuse disorders, with high incidences of cirrhosis, disorders of the liver, traffic accidents, and violent deaths. An ongoing trend is to work at a decreasing number of alcohol-related diseases. Population studies show that alcohol abuse in the Swedish population has been decreasing in recent years. Surveys also show that the great majority of people in Sweden have not (yet) been subjected to alcohol abuse, and that the problem has been handled in a more or less effective way in various settings, including primary health care, where the majority of the population is affected. Patients with alcohol abuse disorders are difficult to reach and difficult to treat. They usually do not use the facilities available, including both primary health care and the psychiatric health care system. Treatment resources are scarce, both by number and quality, and the burden of chronic alcoholism is borne almost exclusively by the individual himself or herself.
